We’re here to provide you with the expert care you need and deserve. To help us provide the best possible experience, be sure to bring the following information to your first visit:
What to bring
- Referral slip and any X-rays from your dentist’s office
- List of all medications you are presently taking (including over-the-counter)
- Dental insurance information, if you have insurance
- Drivers license or photo ID
What to expect
After you make initial contact with our office, we will email you a welcome letter including login information to our secure website, where you can enter your information and review your documents before your visit.
